If your brother is now med free, as I assume from your post, you may want to ask him to try the water treatment. I've been paying attention over the years here and it does seem to work better for those who are not on any meds at the time.
Also, is he using oxygen as an abortive? My husband uses it. He uses welder's oxygen because, at the time, we couldn't get a prescription for medicinal oxygen here. He continues to find success with it and after reassurance from the Poison Centre that it is safe to use, we find it to be much cheaper.
Also, get him to try Dramamine or Benadryl before bed. That can sometimes get him through the night without an attack.
Um...ice! Is he using ice to keep himself calm during the attack and applying it directly to the pain? It helps sometimes too.
These are a few of the things we've had success with and relatively low cost.
Hope you find something that helps him soon.
