by the way I have just finished reading Goadsby and Olesen's definitive book on Cluster Headaches.
My review would go something like this:
"Although it is really no more than a compilation of small studies, this book is a must read for the serious student of clusters. There are summaries of important clinical and epidiological studies that often refute conventional wisdom regarding clusters. Ohter sections offer tantalizing albeit inconclusive theories about cause. My reading of this book convinced me of 2 things:
1 - clusters are real freeking complicated
and
2 - the definitive book on clusters remains to be written ( I think I may be stupid enough to try writing it ).
