I've been thinking the same thing (re:shoulder injury) ... and possible dental work!
8 years ago, i dislocated my shoulder (left) and didn't have it looked at/reset. I let it heal "by itself". Seemed OK. this year i redid the inside of my house - so i had no bed for about 6 months (slept on couch - wrenching shoulder at night).
11 months ago, i had dental work done after which my jaw began to "pop" (left side). Been back to dentist twice - nothing wrong (!!) - but he did hear the "pop" (said the teeth were OK, tho!).
So ... Dec 1997, dental work; Jan 1998, jaw starts to "pop", Feb 1998, start sleeping on couch.
August 1998 - first bout of CH ever. Still in it (that makes it 16 weeks now - am i chronic?). Interesting coincidences.
(hopefully soon i will have waded successfully through the maze of HMO GPs and i will see a neurologist who can give me some drugs for the pain. Having a tough time on asprin alone!) Until then, i have been trying everything to reduce pain. I think I sense a decrease in pain when i roll my head and/or relax my upper back and/or rotate my shoulder (it cracks and pops).
maybe you have something ...

P.S. I have been told herein that it is NOT TMJ (jaw) related, it is NOT back/neck related (e.g. chiropractor will not help). These are the opinions of others on this site - which i respect and value - but with which i'm not so sure i agree.
