I can only address question #3. I've been a smoker for 45 years, a clusterhead for 20 years. In 1985, four years after the clusters started, I quit smoking, and for the next five years was tobacco-free but still a clusterhead. I took up cigarettes again in 1991, still having annual-or-better bouts with CH, until about 1995, when the attacks suddenly stopped. But just this year, the Beast just as suddenly returned. So go figure. For what it's worth, alcohol has never been a trigger for me either, nor has any of the other substances -- MSG, chocolate, caffeine, etc. -- that some people say trigger their attacks.
