Ok, I've added a few more things to the survey. I've also added some test data and a button so you can see how it will look for us.
Please look at the survey and view the results so you can see how it will look. If you have any more suggestions as to what to ask, or would like to see some more choices, please let me know.
We want the survey to show us what we need to see right from the beginning. If we have to change it after we start collecting data, the numbers will be skewed. We definately don't want that.
