There have been some changes made to the Cluster Headache Questionnaire. It was never officially endorsed by OUCH. I own the
copyright. I have removed all references and links to OUCH, CH.com, HSG, and UKCH.
There will be no link to the CHQ on CH.com.
This does not mean your efforts will be wasted. The CHQ will
continue - just under different conditions.
Those of you who worked on it as a contribution to OUCH are now being offered an opportunity to have your names removed from the credits pages. This is your choice, and there will be no hard feelings.
Those who want their logos to remain, please do not email me.
Those who want their logos removed, please email me the word "yes" and identify the logo by stating it's name.
Ex. "yes" Joey
To confirm the deletion you will need to go to the site. I am not posting the URL, but you can check with friends who may know it
or find it in the archives somewhere.
