I will try to answer you:
1) At the first cycle is frequent to have strange lenght, also, unusual durations (the average lenght of an attack is LES than one hour).
There are some cases of start CH in chronic way, but is not so frequent. Anyway, I hope you will not be chronic!
(my cycles lasts 3 months!)
2) My preventative is Verapamil. If you have not big problems with LOW blood pressure, is the best to try first.
Prednisone has a lot of bad side effects, better state as second choice
3) Imitrex: my opinion.... needle or nothing! At least you can try the nasal spray.
O2 is a good choice, with or not Imitrex. read the board for the right flow and mask
