I have received your credit card payment verification, but for some reason (this sometimes happens) did not receive the personal information we need to set up your membership and to send you your membership card. I tried to e-mail you at the address on the payment verification, but the e-mail came back as undeliverable.
Please e-mail me with your address and correct e-mail address. Thank you.
