seems that you have really tried a lot of things, just a suggestion but could be that anxiety is very deep-rooted (with good reason, knowing the headaches i get!) and methods you have tried are great but work on a more superficial level perhaps??? Maybe something else that goes a bit deeper into the emotional response to the pain would help, this is what my partners research is trying to find out. will keep you posted. cheers and all the best.
