Whether it be a doctor or hospital record it is standard practice for the patient to supply the name of the doctor/hospital where you want the record sent, sign a release, perhaps pay a fee.

The key is not to assume this will be an automatic procedure--you must initiate the transfer requrest and sign the release.

SOME doctors will actually give you your file to hand carry to the new doc; this is commonly done with x-ray films, for example.
