Jackie, many of us live in small communities where there is a lack of experience dealing with many unusual medical conditions. This is one of the reasons I started posting the long message, "Cluster Resources", which is now in it's 9th version. (It was last posted about a week ago.)
It has sections on how to deal with difficult doctors; how to find a specialist in headache; titles of several articles and books--aimed at a professional audience--which you could obtain to share with whatever doc circumstances force you to work with. But, primarily, this is material to educate yourself so that you can ask the doc for specific treatments.
The clinical director of the Jefferson (Philly) Headache Clinic has publicly said that upwards of 40% + of M.D.s don't know how to treat complex headache; they haven't had the training and/or there personal attitudes get in the way. Regardless of the cause, we must become more intelligently active in our own treatment and that requires SELF-education.
