Last time I had CH, I was in the same situation. Sometime I needed more then 12mg per 24 hrs. At the 6th week, I finally met my neurologist and he introduced me Amerge.
This time because I don't have a lot of Imitrex injection on stock. I adopt the same tip (2mg per shot). But, I found out 2mg injection will only keep the headache away for about 2 - 3 hours (in my case). I also realize 50mg tablet will last longer (about 6- 8 hours), Amerge will last over 10 hours. So, when the headache starts I take 2 mg injection. After 2 hours if I feel the headache is going to start again I take a 50mg Imitrex tablet or Amerge. Because it takes 30 minutes for the tablet to start effective. Within this 30 minutes, if it is realy bad, I take a 2mg shot or use oxygen for 15 minutes to ease the pain and wait the tablet to start its work. However, my doctor just told me yesterday that Amerge shall not go with Imitrex, supposely. So, if I use Amerge, I will only use Oxygen. With this method, this time (I'm in the 5th week now) I cut down the injection to about 6mg per day or less.
All of these are my own experience. It may not work on everyone and may not be proved by medical professional.
