I've used ercaf for 25 years. Usually works for me in 15-20 minutes. Sometimes doesn't work at all.
Ercaf (cafergot) is not a pain killer, it is a vasoconstrictor. Take too much for too long and the circulation in your fingers and toes will shut down, gangrene will set in and you'll be minus some digits.
Injectable Imitrex works even faster than ercaf. Your bro may want to give it a try. I'm going with it instead of my ercaf next go round.
