My insurance denied my claim for years and then my dr finally got off his butt and wrote the letter and after 2 weeks of paying for it on my own my insurance co finally agreed. I also had threatened to send them a video of myself during a screamer and warned them it wasn't going to be pretty.... I dont know if that helped, I just know the next day it was approved.
