There is a deffinate connection for me as to which side I sleep on. If I sleep on my left side during the night, I will wake up with a ch. But this is only during the episode, after the all clear, it doesn't matter if I sleep on my head.
Also, I have been put in neck braces by some doctors, before I was diagnosed with ch. It was thought that my neck caused the headache. Now I feel sure that the Headaches cause the neck/jaw/eye problems I've had over the years.
