First off, let me say that I know the humiliation and torment of having to go on disability. I have lost several jobs myself - lost everything, and had to swallow my pride and Fight for SSI.

You will have to apply at your local Social Security office. You will be rejected. Appeal the rejection, and you will be denied a second time (This is a standard procedure). Now, the hard part: file an appeal with an Administrative law judge, and find an attorney who does not collect unless you win (they work the hardest for you). Then, you have to obtain all of your medical records proving your condition (Hospital/ER visits; Meds; Jobs lost as a direct result of CH; and a headache journal - showing your frequency, duration and intensity of attacks (see the Kip scale).
It is a long and tedious process, and if you have no safety net or family support - you may take the same hard road I did (lost everything and had to stay with friends.)
Even after you get
