Since the validity of Dr. Goadsby's research is just now gaining acceptance I suspect it will be some years before this question stimulates some research.
Because migraine can occur in preschool age children, it suggests to me that the brain abnormality is either congenital or quickly develops after birth. About 5% of children and 10% of adolescents have migraine with the figure increasing in adults. (Data on cluster in childhood almost doesn't exist.) This progression hints at some developmental processes leading to increasing rates with age.
