This does not seem to be the "classic CH" etiology, but from what I'm picking up here, it is not as uncommon as I thought.
CH being misdiagnosed as something else is entirely understandable, although unfortunate. That is the route most of us arrived here by. My concern here is whether the CH diagnosis might be masking the diagnosis of something else, but for the CH would be more visable for diagnosis and treatment.
