I understand your comments about doing it in public - and I have not done. However, have just had two real nightmare journeys - my train trip is about 55 mins and it's a long time to go when you have got the relief in your pocket. Reckon I could be pretty subtle with the arm. The stomach however gives the opportunity to fold your arms and do it through the buttons of your shirt - very discreet but don't know if it works.
