Your history reads like a carbon copy of mine. I did my bit in 'Nam also, but didn't get CH until '79. Like you, I also waited for Imitrex, but it was a real miracle drug for me--worked within four minutes--then came the angina and angioplasty at Walter Reed. Went through all the meds, some of which can be real nasty, as you know. I was chronic for the first five years, then Lithium (I think) broke the attacks into twice seasonal sessions of about four months. I rely on oxygen almost exclusively. It works for me almost as well as the Imitrex used to. I think others don't get relief because of their methods of use. Perhaps you know all this, but I use a tight-fitting face mask and breathe 100% @ 9 lpm. The prongs in the nose are useless for me. The people on this board will support you and perhaps suggest things you haven't tried. I hope you get some relief and sleep. Feel free to email me if you want to chew the fat.
