I agree with you about trying to ignore the pain. There are times when I will have a mild one start to come on and I will keep doind what I am doing at the time. It sometimes subsides without getting the best of me. Even during my ferociuos beasties, I talk to myself. As if I am talking through the pain.
I however do not agree with the smoking therory. I quit smoking for a couple of years and the attacks did not get any better. Matter of fact, they worsened during that period of time. But alcohol is definately out. Can't touch the stuff. One sip and I'm on the floor talking to myself without the buzz. And that's no fun.
