Hey, Carl! Been reading your posts for some time now, and just wanted to extend my sympathy. Sounds like you have one hell of a cluster, to greatly understate things.
One thing I've found with doctors--it's not just the doctors themselves that can screw you over (under medication, mis-diagnosis, whatever), it's their offices. I've actually left doctors because their offices and staff didn't know the meaning of "customer service." Sorry you had to threaten them to get in, but hope this doc can give you some relief.
