Having used both as injection and nasal spray, I found the spray relatively ineffective. My doc, a neurologist who specializes in CH, concurs. BTW, DHE has been approved for subcutaneous use, making it available for home self-injection (a la insulin). Self-injection may not be your cup of tea, but I've found the DHE injections the best way to stop nighttime headaches without causing insomnia. (I used to use cafergot suppositories, but the caffeine meant goodbye sleep).
The injections can be somewhat painful, but I've found that icing down the injection site beforehand with a reusable ice pack is very effective in reducing the pain.
