As far as intranasal capsaicin hurting....
I had some law-enforcement training a couple of years ago in the use of "pepper mace" (oleoresin capsicum) as a defensive weapon. Part of the training session involved having to take a full-facial close-range hit of 10% OC in a burst spray. This stuff just coats your eyes, runs all the way up your nose, gets in your throat, lungs, hair, you name it. We were warned ahead of time how much it burns and hurts, and that we'd do almost anything to make it stop.
Well, compared to a cluster headache, it was a piece of cake. It wasn't even a "1" on the pain scale. I remember thinking, "Shoot. This isn't so bad."
If intranasal capsaicin will stop a cluster headache, I'll take the equivalent of being maced with OC spray anytime, anywhere.
