For quick treatment of an attack many folks are getting excellent results with Sumatriptan (med literature suggests up to 90% effective with injection method being best for CH). But it is expensive.
DHE as nasal spray is about same price per dose (medical economics drive price).
This explains my bias for Ergomar: it works for me and is about $3/dose. If you have cooperative doc and insurance, a series of trials makes sense to find the least expensive, effective med.
In the long haul, prevention is desirable. Many folks here like Verapamil (and med literature ranks it high) but it takes several weeks to become effective and the need for short term relief continues, hence, the drugs mentional already. One advantage of trying Ergomar is that it can be used for treatment of an attack and as prevention (although for the long haul I personally prefer using something other than ergotamine on a daily basis). In the prevention mode, you use 3 tabs/day and if you watch your total weekly dosing, side effects should not be a problem.
