I suspect this reflects how many docs don't know how to treat cluster/migraine; they don't know the medical literature and so treat the symptom--pain--and not the underlying process.
The number of posts in which folks complain about docs who don't know tells us something.
This is why I take articles from main line medical journals to my doc when I'm asking him for a new med. I accept that he can't know everything (his specialty is rheumatology) but he has come to trust me because I don't ask him to try treatments which don't have some evidence of safety and success.
