Print out the above post by Don C. "Cluster Diagnosis" and take it with you to the doctor. TELL him to read it. INSIST he read it and don't take any BS from him.
Those little exam rooms have corners - back him into one and MAKE him listen to you. First YOU must get HIS attention! Then you get some help.
If you still don't get help - file a complaint on him (Docs don't like to have complaints filed on them). I'm sorry (to the good docs out there) but I get so angry when I hear of a doc just brushing off a patient with CH.
In the meantime drink lots and lots of water. It usually helps most people with CH. Look up Dr. Jerry's Water Treatment in the Archives.
