Your doc is top notch. You are loaded for bear. He left out o2 and lithium. I suggest you just follow his instructions. Imitrex has been a life saver for me (nasal only, no experience with the injections). o2 has helped for months now. I've tried everything on your list plus more. Sometimes they work, sometimes not. You have to try. Good luck.
