In some states, the phrase "medically necessary" has legal meaning. Florida is one where this is true. What this means is that, if your doc writes medically necessary on your script, it pretty much forces the insurance company into approving the meds, even if they are not 'normally' prescribed for a particular situation.
Now, I can't swear this is true, as I'm neither a doc or a lawyer, but it's what my doc told me when I asked him for Zyban to help me quit smoking. He wrote it, my insurance company approved it. Too bad it didn't work.
