I have found that 1) being a pest is the best thing to do when you're hurting, and 2) writing a very concerned, sincere letter of complaint to the doctor about poor service sometimes helps prevent future occurrences. Since they read their mail (not e-mail!) during less hectic times, it tends to stick a bit.
I was a hospital administrator for many years and have observed both arrogant, uncaring doctors and sloppy staffs at their offices. Both seem to be in the minority, but for some reason the neurologists and cardiologists are skewed toward this type of behavior. If your doctor thinks MD stands for "Minor Deity", a reprimand won't work and you'll need to find a new one anyway. Otherwise, he or she may respond really well.
In either case it can't hurt!
