I know exactly what you're going through. I'm usually a healthily normal social being, not afraid to say what's on my mind, noramlly assertive and aggressive, but I come before the presence of a doctor, I turn into a sort of passive ectoplasm , practically unable to utter my own name much less actually challenge or question "the doctor presence."
Twice now I've written long letters to my doctor, laying it all out in polite, but blunt terms. I know it might not be the "right" way to do it, but it is a way, and the air is cleared and hey, you're not taking up their precious office time.
If all else fails, it's worth a try, and it has worked for me.
