Hi,
I'd send her a copy of your post that you sent all of us; it pretty well lays out your feelings and if she doesn't get the picture for it, fire her! Write her a letter and fire her. She works for you. shes not "doing you a favor" by being your doctor, you hired her to do a job and if she's not doing it, fire her.
Then when you are interviewing new doctors, have a talk with them about what your expectations are and ask them if they can meet them.
