It's not uncommon for insurance companies to not pay for O2 at first request. But I haven't heard of them not paying for triptans. Most migraine meds are usually covered, I thought. If you can find out the reason for their denial and let us know, maybe we can help you with the info you need to win your appeal.
I don't know why their is no CH-specific code on insurance forms.
