I referenced Goadsby's hypothalmic activation article below (it's in the OUCH Library). The hypothalmic region activates before the vasodialation occurs. That's why it should be called a neurovascular headache rather than a vascular headache. The pain not only comes from the dialation of the blood vessels but also from some action on the trigeminal nerve.
