due to technical problems, i.e. my lack of training in this area of endeavor.
When my clever advisors get me straightened out, the survey will be published - temporarily on my Web site, later perhaps as an OUCH-sanctioned questionnaire.
Everyone will be able to list any and all medications and their effectiveness.
Any advice about setting up such a questionnaire,
formatting questions, tabulating the responses, and preparing reports will be appreciated.
