When local resources have been exhausted it's time to ask one or two of the docs she has seen for a referral to a specialty medical center. There, shewould find a deeper concentration of skills in the needed specialty.

I  would run it by your new neuro at UCSD, maybe his team would have some ideas, or even want to take on the challenge.

Totally agree with the suggestions. This is not something we can diagnose as that needs a suitably qualified and experienced doctor to review the case in person.
