Tell me about verapamil

Please tell me about verapamil.  Just got a prescription and take three 80 mg tablets a day.  Pain free for six days and got a 9 tonight.  should i take more, what time of day and what dosage?  Now taking three spread out through day.  Thanks for your help.

Peking,

Do not change your dose without doctor approval... At 480mg and above I believe you need an EKG to make sure your heart can take it.

Hi and welcome. Roland said it when one does NOT increase or decrease meds dosages without your doc's approval. Remember that Verapamil takes time to get into your system, up to a couple of weeks.

I was on 240mg Verap daily last cycle but it was too low for me. My neuro has me on 480mg daily now and it is working pretty good so far. I take two 80mg's three times daily with food.

This is my preventative med, what are you using as an abortive? I, along with most here use o2.
100%o2 with at least a 15 lpm regulator with a non-rebreather mask. Get on the o2 at the very first sign of an on-coming hit. No outside air what so ever! Have you talked to your doc regarding o2? It is my miracle in a green bottle!

Another note on Verap, be sure to take some form of fiber cereal or similar as it will cause constipation and a great deal of pain in the ole' stomach.

Individualizing treatment with verapamil for cluster headache patients.

    Background.-Verapamil is currently the best available prophylactic drug for patients experiencing cluster headaches (CHs). Published papers usually state 240 to 480 mg taken in three divided doses give good results, ranging from 50% to 80%; others mention higher doses-720, even 1200 mg per day. In clinical practice we found we needed to adapt dosage to individual's time of attacks, in particular giving higher doses before going to bed to suppress severe nocturnal episodes. A few only required 120 mg daily. We therefore evolved a scheme for steady and progressive drug increase until satisfactory control had been achieved. Objective.-To find the minimum dose of verapamil required to prevent episodic and chronic cluster headaches by supervising each individual and adjusting the dosage accordingly. Methods.-Consecutive patients with episodic or chronic CH (satisfying International Headache Society (IHS) criteria) were started on verapamil 40 mg in the morning, 80 mg early afternoon, and 80 mg before going to bed. Patients kept a diary of all attacks, recording times of onset, duration, and severity. They were advised, verbally and in writing, to add 40 mg verapamil on alternate days, depending on their attack timing: with nocturnal episodes the first increase was the evening dose and next the afternoon one; when attacks occurred on or soon after waking, we advised setting an alarm clock 2 hours before the usual waking time and then taking the medication. Patients were followed-up at weekly intervals until attacks were controlled. They were also reviewed when a cluster period had ended, and advised to continue on the same dose for a further 2 weeks before starting systematic reduction. Chronic cluster patients were reviewed as often as necessary. Results.-Seventy consecutive patients, 52 with episodic CH during cluster periods and 18 with chronic CH, were all treated with verapamil as above. Complete relief from headaches was obtained in 49 (94%) of 52 with episodic, and 10 (55%) of 18 with chronic CH; the majority needed 200 to 480 mg, but 9 in the episodic, and 3 in the chronic group, needed 520 to 960 mg for control. Ten, 2 in the episodic and 8 in the chronic group, with incomplete relief, required additional therapy-lithium, sumatriptan, or sodium valproate. One patient withdrew because verapamil made her too tired, another developed Stevens-Johnson syndrome, and the drug was withdrawn. Conclusions.-Providing the dosage for each individual is adequate, preventing CH with verapamil is highly effective, taken three (occasionally with higher doses, four) times a day. In the majority (94%) with episodic CH steady dose increase under supervision, totally suppressed attacks. However in the chronic variety only 55% were completely relieved, 69% men, but only 20% women. In both groups, for those with partial attack suppression, additional prophylactic drugs or acute treatment was necessary. (Headache 2004;44:1013-1018).

i am on verapamil -Started at 80mg 3 times a day and it didnt help much
Now my headache log looks like this
dec11 --seen dr bryans today -increase verapamil to 160mg 3 times a day
     --took relpax 800pm
de12  --took relpax 1000pm
     --went for blood tests

de13  --mild headache most of day,getting worse at600pm took relpax

de14  --
de15  --mild headache all day -took repax at530
de16  --
de17  --
de18  --
de19  --
de20  --
de21  --
de22  --
de23  --ct scan

AS you can see,a week plus,with no full blown attack.

Welcome to the board Peking, not much to add but just to repeat 3 things.

Don't ever increase or decrease meds without working with your doctor. These are some pretty high horsepower meds we use.

Don't be suprised if it takes up to two weeks for a preventative to become effective. I use lithium and it takes 10 to 14 days to reach therapeudic levels.

The best battle plan against these involves a 2 pronged approach. A good preventative combined with an effective abortive. If you haven't tried oxygen yet you really should. It's enjoying a better then 70% success rate, is cheap, no side effects, so certainly worth a shot!

The responses and support here are amazing.  At my K7s and below, I go right to this website during an attack; 8 and above, I can't for at least a while.  Thank you so much for all the information.  What a wonderful place to have -- especially in the middle of the night when I am so alone and scared.

My Neuro started me on verapamil 240mg about 2 weeks ago, but in ONE pill.  The info Ive seen here shows a divided dose, taking a smaller dose 3x daily.

Anyone here taken one mega dose daily, and seen success?  So far I have had two nights without any attacks since starting, and getting 3-4 per night on the other nights.  They are much more mild, thanks to the O2 also i believe.  So its hard to tell if the verapamil is even working, since it takes a few weeks to build up.   Given that my cycles only last 4-5 weeks, this would make it hard to fool with different doses with enough time to judge the effects i would think..?

The studies have been done with regular release verapamil.  Some docs prescribe the Extended Release.  Gary's on ER - not seeing any benefit yet, but just recently went up to 360mg.  Prior neuro had him up to 480 and saw no benefit.  But this neuro wanted to go slow at first.  Can go as high as 960mg - 1200mg for therapeutic treatment of cluster.

That's how I found this crew, in the middle of the night.  Sometimes I have to make myself come away from the computer cos the screen hurts my eyes and I get the shadows. Smiley
It sometimes goes the same way when I'm reading for any length of time.
Anyone else notice this? Huh Undecided

Man I hate the Verap. Really makes me feel like crap. I avoid it at all cost. I think heroin might be better for my body........
