Title: Any ideas on what this might be...UPDATED
Post by catlind on Mar 27th, 2008, 10:05am
My daughter started with a headache about 6 p.m. last night.  By 8 p.m. she was bent over sobbing in pain that her head hurt and she wanted to gouge her eyes out.  I sent her to bed with an Aleve and Tylenol (she's only 14).  She was writhing around in her bed sobbing in hysterics screaming her head hurt and her eyes hurt and her neck and back hurt like someone had stuck in a knife in it.

Off to the ER we go.  They gave her pain meds, ended up having to give her 2mg of dilaudid to ease her pain.  After blood work and a CT, they had to do LP (spinal tap) as they were concerned it might be meningitis.

All the while my baby girl is crying and saying Mommy am I gonna die, I feel like I'm dying, I don't wanna die.  Talk about heart pain.

In the end, the tap was clear, no meningitis.  They ruled it migraine.

Does ANYONE here know of any kind of headache that would cause EXTREME neck and back pain?.. I mean pain that made it so she couldn't move, and she has a high pain tolerance.

Any info would be appreciated.  She's still sleeping and I"m waiting to see if she will suffer a spinal headache at which point we'll be off to the docs again.

Thanks again folks

Cat

p.s. any and all vibes, prayers, mojo and energy is much welcome.

Could it be something viral Cat? Just thinking, trying to think of any possiblity.

Poor kid, that's horrible and poor you too. I hope she is feeling better soon.

I was thinking the same thing, viral meningitis.  Bacterial will show in the spinal fluid as cloudy, much like cloudy urine for a bladder infection.  Viral does not show.  (I had meningitis at her age myself)

Is she running a fever?

Also, sometimes the bacterial form takes longer to show in the spinal fluid even if it's there.
I'd 1) get a second opinion, 2) keep on top of this, and do NOT let her around other kids, just in case.

What Redd said.

My mom had viral meningitis and the Dr's took forever to correctly diagnose her.

Definately get 2nd opinion if she is not feeling better.

My thought is that they probably should have put her on anti-biotics just in case if there was any kind of fever along with the head pain.  This is an odd question--but did she pick up any reptiles (certain kinds I'm not sure which kinds--I think boa constrictors)--they can pass meningitis infection (bacterial).
If no fever, I would guess they are right with the migraine diagnosis but still if you have any question, maybe call your doc and ask for anti-biotic.
Let us know how she is doing.  Star

Basilar migraine involves the brain stem, and it can cause problems moving, slurred speech, etc.  Not sure how often it spreads to back/neck pain.  Basilar migraine seems to be relatively common among adolescent girls.  If it is basilar, triptans are a bad idea - could cause big problems.

Thanks everyone.  She is actually on Amoxicillin right now for a sinus flare that she had about 9 days ago now.  She was sick a week ago Monday - the 17th and I had her in to the pediatrician.  They tested for influenza, strep and mono, all negative, so with the inflamed sinuses put her on an antibiotic.

She has been running a fever, and she has displayed the following symptoms over the last 2 days (including yesterday before the head/neck/back pain hit): Unusual fatigue with episodes of fainting and passing out, the severe headache, worst ever, change in vision, stiff neck, areas of numbness and tingling and weakness on her right side - at one point her toes were completely numb while in the ER, as well as stumbling.

I have an appt with her pediatrician who is, IMO the best in the country.  She has saved my kids lives countless times.  Unfortunately their computers are down and they can't book an appt for her, so the Dr's nurse is going to call me as soon as they can get her in for an appt.

She is still sleeping soundly, I keep checking on her to be sure she's breathing.  I know what a spinal headace feels like, so I'm on pins and needles waiting to see if she fares better.

There is nothing more heart wrenching than when your child looks at you with utter terror and asks you if they are going to die.

While I do not know what the cause of this is yet, all your suggestions are very helpful as I can take them to her ped. and go from there.  Please keep them coming.  I had no idea viral meningitis showed as clear fluid.  She is off school until we can sort out what the problem is.

Thanks again, and please keep the suggestions coming.

Cat

Edited to add:  Normally she wouldn't have been given antibiotics for a sinus inflammation but after her severe and protracted bout with pneumonia in the fall, her pediatrician is extra careful and does NOT want to take a chance with her health.

The anti-biotic could also be masking the extent of a bacterial meingitis, which also can help it to mutate and become drug resistant.

The sleeping and fatiuge is consistant with meningitis as well.  (I slept 4 weeks of my life away when I had it.)

The numbness could be basilar migrain, or it could be blocked nerves due to the stress and tension as she tightens up and fights against the pain.  No knowing for sure.

If it's a matter of another day to get her in to her regular pediatrician, I'd take to the ER and ask for her to be admitted.  Sleeping alot can also mean she isn't getting enough fluids (can drink when sleeping) which will make any of her pain worse, and help spread whatever infection might be there much faster?

How have her eating habits been?  I am asking because you mentioned the fatigue and passing out.

I am probably wrong, but if she is not eating well, and also sick from a sinus infection, that could cause the fatigue and fainting.  It may also cause the headache.

I think no matter what I would keep trying to find a doctor to help her.  If it is one of the conditions the other posts talk about, it needs to be treated.  I had to go to many doctors before I found one that would take me seriously with the problems my son faced.  I even had one doctor tell me to quit feeding him because he would be nothing more than a vegetable if he lived to be 2 years old.  He is now 13.  He had aspiration phnuemonia shortly after birth, and it was not caught in time.  He now is left with Cerebral Palsy, speech disorder, and delayed learning.  He is, however, in the 8th grade (he was never held back).  He has a teacher aide who helps him alot.

My point that I guess I was trying to make with the story of my son was that it was worth it though to see him today, happy and full of life again.

Best wishes for your daughter, and you too.

Just as a precaution I would not rule out that this might also
be drug related.
Amoxicillin can cause headaches and severe allergic reactions.
I have experienced severe headaches, sweating and rash
while taking it.

Cat,

I will put out energy.  Get her in to the doc and talk about every. Let her expalin each and ever little thing with the  doctor.  Show the doctor how far down the pain was going in her pack and where it moved around in her head.

The headache and back pain is similar to what I get when I trigger the Occipital Neuralagia. But those typically trigger a Cluster after that so there is not a way for me to say where the pain is after that.  But I've never passed out from it. The fatigue I feel after a Hit is because my whole body tenses up and I feel like I went 2 rounds in the UFC.

Cat, follow your gut.  You have great intuition.  While you might not be able to fix it, you know how to focus, to make sure that she is safe.

Call me if you need to.

Blessed Be, my Sister.
Tia

Cat try a ear nose and throat specialist. It could be that sinus is not clearing up. Believe me sinus makes your neck, ear, eye hurt. It can be really painful. I had a sinus infection and they treated it. The meds did not work. I finally got it fixed by going to the specialist.
I will be praying for her.

Thank you so very much everyone for your input and suggestions.  It has helped tremendously.

We got home from the docs this morning, and they retested for flu, strep and mono, she came back as slightly elevated on the mono, although the tech put it as a negative, but the Doc feels that isn't accurate and wants to consider her a real candidate for mono once we rule out the kidney issues.....then they did a urinalysis - the doc seemed concerned about her kidneys.  One of the reasons I LOVE my pediatrician is because she doesn't candy coat anything and gives the you the abc's of treating WHATEVER the outcome is.

There was blood in the urine as well as another chemical that she said can indicate early kidney failure.  They are running further tests but they take 24 hours so the doc will call us tomorrow with the results and let us know if she needs to go in the hospital.

She is sleeping peacefully right now, they gave her a shot of Rocephin (sp) - a very strong anti-biotic as well as a shot of toradol to ease her pain.  She is still suffering back, neck and head pain but not as severe.

So the drama carries on at the Lind household.  It never ends does it - everyone of us, as soon as we think we get a grip on things, someone adds oil to the rope and watches us slide.

Thank heavens for knots and clusterbuddies and second Mom's.  I love ya Jacks.

Damn, Cat!  Sending you all the strength and good luck I have.  Keep us updated.  Now, if she does have a bad enough sinus problem, couple that with a migraine and if she's trying to hold her head still, she could be having muscle spasms in her neck and back.  I've had muscle spasms bad enough that it would take my breath away.

I just spoke to Cat, her daughter has been admitted to one of the local hospitals. They seem to believe that her kidneys are the source of her problems. More tests should be able to tell exactly what's going on.

She asked that I post an update and I ask that you keep Cat and her daughter in your thoughts.

Just got home from the hospital, had to leave or risk jail time for homicide of a physician  >:(

They are running every test under the sun, and have yet to find a definitive explanation for all her symptoms.  Get this, her back and flanks (kidneys) hurt EXTREMELY bad, her neck hurts too, so they did a head x-ray. HELLO

They have her on fluids, 4 bags of fluids, she has pee'd once in 24 hours now.

She is in extreme pain, now I can deal with my CH pain, but I cannot deal with my baby girl in that kind of pain.  And the mother fucker doctor won't give her anything except toradol.  I understand that is a logical med, however, when it ISN'T working, you do something else, you DON'T LEAVE A FUCKING 14 Y/O WRITHING IN AGONY AND SOBBING.

Ok, rant over.  Clark has gone up to the hospital to be with her, I had to take a break or risk breaking bones on medical staff.

If they haven't sorted out her pain and gotten some answers tomorrow a.m. somebody is going to discover just how fucking hard Cat's 2x4 is................

Your support, thoughts, vibes and prayers are very much appreciated, I am very grateful.

I had a severe kidney infection and stones some years ago and the pain in my back and specially lower back was excrutianing, the test also showed blood in the urine and it was very painful to pee. High fever and lots of pain for lots of days but my Doctor put me in special medicine to cure the infection and to break the stones so I did not need surgery. Damn Cat, I do know how you feel, though.  After 7 surgeries with Sarah, it still kills me to see her hurt.

I know that they've probably ran this *test*, but did they run any tests for encephalitis?  When I read your original post, describing the symptoms, it near about drove me crazy, trying to remember what it could be.  So, I had Jimbo read it and he asked if she'd been tested for encephalitis.

Keep us posted.

Good thoughts and strength coming your way.

Title: Re: Any ideas on what this might be...UPDATE
Post by catlind on Mar 31st, 2008, 9:43am
Ok, saw her pediatrician this morning.  GAWD I love that woman.  She is the only sane one in Childrens hospital I think.

They have now run every test they can think of to rule out meningitis, kidney disease, adenovirus and a host of other viruses, strep, flu, mono, etc etc.

Doc Whitcomb came in and said this just doesn't add up.  We are going to do an ultrasound on her kidneys today to rule out stones, then we are going to do an MRI of the spine to rule out growth or tumor of the spine.  While viral meningitis will not be affected by her being on strong antibiotics, bacterial can be, but she has none of the other markers.

Carolyn, encephalitis is the word of the day. Apparently the enterovirus (sp) is a very virulent virus and results in encephalitis if not treated promptly and has all the hallmark symptoms she seems to be experiencing, so that is on the agenda for today as well.

If all of those come up negative, then she is going to start down a darker path.  She shoots straight, she never sugar coats and that's why I love her.  If Andrea is indeed in danger of a much more dangerous illness, I would rather know up front what I am facing.

I may be able to bring my laptop up to the floor later so I can post as we learn more.  I am taking a break as I was up all night with her, and Clark is going in to spell me off so I can get some rest.

I can't say I've felt more helpless in my life.  Watching that child lay there crying in pain and knowing that all the hugs and love in the world can't make it better.   I miss the days when band aids and cookies fixed everything :(

Thanks for all your support, any ideas or suggestions please keep them coming, my doc is interested in ANY theories as we just are at a loss to explain all the symptoms.

Ok, well kidney ultrasound was normal, she just got back from the spinal MRI and we had the consult with neurology......

Ok, here's where I may end up in jail.  The neuro's take?  He sent a shrink in.  Says she needs a psych consult and the pain is a spinal headache.  HELLO DUMB FUCK, she had the headache BEFORE THE FUCKING SPINAL TAP.  I held my tongue, BARELY, and waited to talk to the pediatrician this morning.

She agreed with me whole heartedly that the neuro is just plain odd.

We are getting the run around from the other hospital that did the LP and can't get a clear answer on whether they tested for enterovirus (the virus that causes encephalitis) and if they kept a vial of CSF in reserve.  If not, she wants another tap, but of course the neuro has now said it's all in her head and she doesn't need one.

Needless to say I'm about to start going ape$hit on some medical staff.

Anyone wanna start a collection for my bail bond?

Will update as I can.

Thanks everyone for all your thoughts prayers and vibes.  It helps immensely.

Right now, all signs are pointing to encephalitis but it's whether or not they will allow the testing to PROVE it.

Ok, I feel better now.

Clark is at the hospital with her in his nice calm manner and keeping Andrea calm.  I am at home attempting to regain some composure before I go and commit a felonious act.

WTF is so hard about giving a kid pain medication to help her stay comfortable???  Oh right, the neuro says she's a psych case.

Cat,

I am so sorry hun and will do what I can on this end.  See if we cant get that pain directed elsewhere.

I wish I remembered what the hell I had beck in college.  All I remember is I went from I'm fine to waking up in the middle of the night screaming in pain.  I had a high fever, was doubled over with pain in my lower back and abdominals and my head felt like it was going to explode.  I had 6 different doctors trying to figure out what the hell it was. After 3 days they put me on a morphine pump and I had oral Pain killers I could take.  I was in the hospital for 10 days, they released me and the doc forgot to prescribe me anything, antibiotics or  pain meds.  4 hours later I was back in the hospital in the same shape I was in the first time. They kept me for another 2 weeks.  They still had no idea what was causing it, but finally decided that it was some sort of viral infection that had to run its course.

The funny thing is that I drove myself to the hospital the first time, and walked well about crawled into ER.  I got reamed by the docs for doing that.  When I went back in the 2nd time, my roommate refused to drive me,  so I called everyone I had the number for to see if someone would drive me.  Shaun (Who was married to a girl in one of my classes) took me to the hospital and sat with me for 8 hours until he had to go to work. I was so f'ing stressed and freaked out about being back because they did not know what was happening.  3 months later his wife left him and a year after that we got married.

The only thing they knew was that it attacked everyone of my female organs, my kidneys and liver. But once it ran its course I was not in pain anymore. I was fatigued for months.  All I know is I felt like I was going to die.  But they never had a name for it.  Had no idea how I caught it or what made it go away.

Call me if you need to. You know that I will help in any way that I can.

Love and Light, my sister
Tia

Good or bad news I have no idea....Andrea was discharged tonight at 9:00pm cdt.  UGH.  No one there could answer the question as to whether or not she actually has EVEN BEEN tested for enterovirus (the one that causes encephalitis) let alone if it is positive or negative.

I'll be on the phone tomorrow at the crack of dawn to her pediatrician to find out what the fuck happened.  It was the on call neuro and pediatrician that discharged her, her pediatrician was wanting to wait until TOMORROW not tonight.

So my experience with Children's hospital so far is that they will allow - AGAINST HIPPA LAWS - law enforcement officers to demand a discharge in order to arrest and cuff a minor despite further testing for known seizures, and they send minors who are quite obviously very ill and in excruciating pain to be discharged in damn near the dead of night.

I'm gonna have some real fun with an accreditation committee over this one.

NC better duck, I've got one fuck of a 2x4 that is about to start swingin...........

Cat, I am so sorry your daughter is having such a rough time of this. And you too.  [smiley=hug.gif]

One thing is, if the hospital seems to be clueless about giving your daughters meds, especially pain meds, and they don't want to call your doc, then YOU call the doc. I've had to do this before, and the first time I told them I was going to do it, if they wouldn't, they blew me off. The doctor called them within 5  minutes after I got off the phone with him. The next time I told them, they made the call immediately. Nothing wrong with you calling the doctor.

I can't believe they would discharge your daughter without consulting her pediatrician. She is your ped's patient, so WTF?

Anyway, I am hoping this will run it's course fast, whatever it is, and your daughter gets healthy and out of that horrible pain. Take names, you'll need them. And you take care, hon. I know this has put a major amount of stress on you and your family.

Well the good news is she is in better spirits this morning for being at home in her room with all her things (her room is something else OY).  I just got off the phone with the docs office, her ped is off on Wed. but the nurse took down all the info and they are going to call her and get back to me before noon.

I think the one who is happiest of all to have us home is Clark.  Poor guy, he's been holding down the fort at home with Alex and Ayden while I've been at the hospital with Andrea.

One way or another, I will have an answer to the enterovirus, and then I will know.

All things for a reason right? ........... oy

Thank you all, you have been a rock of support and help to me through this and I can't thank you all enough.  My gratitude is huge.  I will most definitely pay it forward.

This whole thing is some kinda worst nightmare, Cat. I admit, this is the first time I have read about what you and Andrea.and Clark..et al.have been going through. At first blush, I thought Andrea was suffering the same virus I have been.....the pain, back, neck, head, hips,....and it HURT......but not like you have described! And not such protracted pain! Besides, I'm on day 6, and I think I'm gonna live! (tanks Gawdd!)
I cannot imagine a Dr allowing a 14 yr old to suffer so much.......and, for the life of me, I cannot imagine the possibility that NOTHING can be found?
I have no answers- but I have hope, and I have prayers and fervent wishes that Andrea is back to herself in no time..........and I have hugs for you............Mom isn't the easiest job in the world, is it?
And, if that bat needs to be wielded......I'll help with the defense....let us know what bank to send it to,K?
I'll be watching and hoping for updates..........positive ones, and soon, till then, juju, boji stones, amulets......whatever I've got are yours.......coz, it's hard to wring my hands and type at the same time.........
