Can someone please post the link again for the form that you doc fills out for ER treatment? I knowt hat I saw it and printed out a frw, but I go see my doc Monday at 8:15 amd I woud like to have him fill it out. I think that will help some as well, cause it will force hi to make a decision regarding my treatment and not be so wishy washy.
