Cluster Headache forms for Doctor and Patient:
Hopefully these will spare someone the suffering and humiliation that can go along with an ER visit.

You will have to do the following because you must specify patient/physician and use a signin process
http://www.headaches.org/
Choose "Headache Sufferer"
Choose "Educational Resources" to the left
Scroll to the bottom "Emergency Room Treatment Forms"

It just dawned on me, it might be wise to sign up with the National Headache Foundation. http://www.headaches.org/consumer/registration.jsp By signing up I become another statistic with CH. Thus, improving the statistics of those with CH. If enough of us sign up, it might be considered an epidemic, allowing more grant money to flow :)
My previous post basically said to dismiss the signup.
