Title: Survey: Cluster and sleep + start of episode
Post by cluster on Apr 21st, 2007, 1:30pm
Dear all,

this gentleman has a couple of questions.
I think we should support him with answers.

http://www.uke.uni-hamburg.de/institute/systemische-neurowissenschaften/images_content/institut-systemische-neurowissenschaften/may.jpg

Priv.-Doz. Dr. med. Arne May

Dr. May is one of the European experts on headaches, especially cluster headaches. (He works here.) (http://www.uke.uni-hamburg.de/institute/systemische-neurowissenschaften/index_24920.php)

I'll put his questions into a reply to this posting. Please be so kind, click the "Quote" option on the upper right corner of the following message and fill in your answers.

Thanks in advance and painfree wishes,
Friedrich

Title: Questionaire
Post by cluster on Apr 21st, 2007, 1:35pm
Are you chronic or episodic?

> Chronic

What is your age?

> 44

Are you male or female?

> male

For how many years have you had cluster headache now?

> 6

During sleep:

Does the pain wake you up?

> Yes

Or do you wake up before the pain starts?

> Yes

If you answer both of the last two question with “yes” (both happens) – what happens most of the times? Perhaps you can give a percentage, of what happens most.

> 90% the pain wakes me up

Please answer only if you are episodic:

Does an episode start for you with the first attack of pain?

>

Or can you feel somehow in advance that the episode is about to start again?

>

Or have you experienced both?

>

Are you chronic or episodic?

Episodic

What is your age?

47

Are you male or female?

Male

For how many years have you had cluster headache now?

31

During sleep:

Does the pain wake you up?

Yes (a recent development. For less then 10 years,  prior to that no wake up hits)

Or do you wake up before the pain starts?

No I dream about getting a headache, don't actually wake up until the head ache is establishing itself. This coincided with a changing sleep cycle as I've aged. I use to sleep like a zombie, nnothing could wake me up. As I age I sleep much more lightly.

If you answer both of the last two question with “yes” (both happens) – what happens most of the times? Perhaps you can give a percentage, of what happens most.

Please answer only if you are episodic:

Does an episode start for you with the first attack of pain?

>

Or can you feel somehow in advance that the episode is about to start again?

>

Or have you experienced both?

Both, but most of my cycles start with an almost panicy feeling of "oh no they're back" Occasionally they will start with a KIP 3 or 4 that comes from out of the blue.

> Yes, I wake up like clockwork with no pain (fear?)

If you answer both of the last two question with “yes” (both happens) – what happens most of the times? Perhaps you can give a percentage, of what happens most.

> 65% the pain wakes me up.  The more thought I give to the question the higher the percentage should be.  I would say it is more like 90% the pain wakes me up.  In my last cycle being awakened by fear was more prevalent at peak of the cycle.

Please answer only if you are episodic:

Does an episode start for you with the first attack of pain?

> Yes and No.  Not necessarily, shadows and an episode are two different things to me.  Shadows are pain of one type or other.  Anything less than 5 minutes in duration I would not consider an episode, yet, it is pain?

Or can you feel somehow in advance that the episode is about to start again?

> I think that clarifies the previous question.  I cannot say whether the shadows will lead to a full blown episode.  I can say there are certain indicators one might be coming, and not only coming, but quickly.  With me, it can be one way or another.  The sensations (tingling in the nasal passage, the dull pain, the sharp stab in my eye, clogged nasal passage) are some indicators, yet they may be shadows (the affliction letting me know it is not gone).

[/b]Or have you experienced both?[/b]

I am sure we can add to the questions being asked.  Ask the Doc if he would like to hear some questions we think could be useful?

Hello Donnie,

thank you (and everybody else who answered so far too) very much for your answers.

Perhaps we can start another thread with patients questions?

This is just a "quick an' dirty survey", Dr. May has his reasons to ask these specific questions. You know the surveys on the left of this page?

Thank you all for your answers,
Thank you Bob for the link,
Thank you DJ for this dear place and message board and the CH.com surveys on the left side of the screen.

Bob, the link doesn't work, these are the two CH.com surveys, they are great.  @Everybody: Please fill in your answers, if you haven't done so yet:

http://www.clusterheadaches.com/survey/index.html

http://www.clusterheadaches.com/meds/index.html

Since 2005 the counters of both surveys haven't changed. Has nobody bothered to answer them? (Just a question.)

This free text survey here is just a quick one to answer the questions from Dr. May. I'll wait one more week for more answers and then I will summarize the results, post them here and send them to Dr. May.

Anybody who would like to participate: Just click the following link to answer the questions:

Title: Results
Post by cluster on May 14th, 2007, 4:45am
Thank you all!

A brief summary of the results:

29 people answered here at CH.com + 14 answers from the German CK-Wissen website = 43 answers in total.

Of these 43:  30 are episodic, 11 are chronic and two are suffering their first episode. 34 are male (incl. one "sort of")) and 9 are female.

Average age is 44, range from 20 to 61.

The 43 responding people in total have suffered CH for 641 years, average 15 years, range 8 months to 40 years.

Age at onset was 29 as the average, range from 12 years to 52 years.

One person does not have nightime hits and yorkshiremen don't sleep during episodes? (Sorry Yorky, I shouldn't be "yoking" about this.)

Of the 41 remaining answers two persons always wake up before the pain starts -  17 always wake up caused by the pain and 22 experience both. The percentage of "pain wakes me up" given by the 22 who experience both is "82% the pain wakes me" up as an average, range is from 40% - 95%.

No real surprise, is it ?

Episodics:

Does an episode start for you with the first attack of pain? - 30%
Or can you feel somehow in advance that the episode is about to start again? - 23%
Or have you experienced both? - 47%

I will sent these results to Dr. A. May today - and hope this helps to find the cure.  :)

Again, thank you all for participating and pf wishes,
Friedrich

(edit to get the numbers right)

I find that I'm waking now just before the real pain starts

No, for me the twinges signal the on-set.

For how many years have you had cluster headache now?

During sleep:

Does the pain wake you up?

> Yes

Or do you wake up before the pain starts?

> No

If you answer both of the last two question with “yes” (both happens) – what happens most of the times? Perhaps you can give a percentage, of what happens most.

> 90% the pain wakes me up

Please answer only if you are episodic:

Does an episode start for you with the first attack of pain?

>Yes - ache in right sinus area
Or can you feel somehow in advance that the episode is about to start again?

Or have you experienced both?

Hello my dear "chronic 50 yr old female, CH 1/2 of my life (that's 25 years using the new math)pain used to wake me, but am taking enough sleep stuff to knock out a rhino. never relax-never rest-NEVER NAP!!! ;;D "

Thank you very much! (Kathy?)

Perhaps I'll sum the additional answers up when I am retired in about 20 years. - If somebody else wants to do that earlier than me, I won't mind. The evaluation done above is with Dr. May.

Thank you very much to everybody who answered the questions!

btw: In 1952 E. Charles Kunkle wrote: "In two-thirds of the patients the attacks always or commonly began during sleep." - It is still the same, isn't it?

EC Kunkle coined the term "cluster" headache. In his 1952 article the word "cluster" is always in " ":
