Title: Cluster anomoly Post by CJohnson on May 29th, 2003, 2:19pm Lately I have been wading through a veritable sea of information about CH which I've found in the post archives, the OUCH website and other places on the internet. Here is what I think I've learned so far. Please correct me if you notice I am wrong about something. An irregularity in the hypothalamus generates an anomaly in some aspect of circadean rhythm which causes serotonin modulation to become unstable. Somehow, some aspect of this serotonergic instability stimulates the trigeminal nerve. Thus activated, C-fibers within the trigeminal nerve release vasoactive neuropeptides (such as cytokines), calcitonin gene-related peptide (CGRP), substance P (SP) and neurokanin A, The released CGRP causes the trigeminal nerve to activate again and the blood vessels to dilate abnormally. Some element of this mechanism generates (or fails to prohibit) signals which travel, unmodulated by the spine, through a system of neurons to a part of the brain, which interprets them as CH pain.
