I read a story about a hiker in Utah who had a 200 pound boulder fall and pin him to the ground. After a couple of days with no food or water, he amputated his arm to free himself. http://www.cnn.com/2003/US/Central/05/02/climber.amputation.ap/index.html

No pain meds at all. I bet even Jonny doesn't have balls that big. :o

Must have been some pretty intense pain but I wonder how would he like to cut that arm off 3 or 4 times a day for about 4 months.

I've heard tales about guys that chew their own arm off....something about "coyote arm".... ;D

That's actually quite different because it's usually a direct result of excess alcohol consumption the night before ;D
