Yes it does exist, and has existed for a long time. However since there is no link to it ANYWHERE (including on the shroom sites) nobody can ever find it!!!

Would someone from either "Cluster Busters" mind emailing me back - I emailed you about 6 months ago  
The responses are emailed back to myself, pinky, and earth from EROWID. Once we get ANY responses back (apart from our own) we might even publish some results.

I just filled out the survey and got the following error msg upon hitting submit:
404 Error (File not found)
I have no idea if my survey results took or not.

Karla, that either means the page you get sent to after submitting the form has been moved or changed.
OR it might mean that the browser was unable to find the form 'script' on the server.
Whoever maintains it might need to check the code/links and make sure everything is current.
FormMail.pl is a REALLY easy to use form script that is freeware.

Oh Flash...yee of little faith! 
Yes the link does exist on one of the mushroom/clusterbuster sites.
Its on the FAQ and has its own section....2.2.2 I believe.
Its been visited by the same number of people as the website so it seems that everyone is checking out both sites if they check out one.
Granted....it should have also had a very visible place on the website. I will take care of that asap.
