Don't believe you have posted this. Good looking material which, while I have not read every page, appears to duplicate a 2-3 other sources of same material/length.  
 
Personally, I recommend a headache doc who runs: http://headachedrugs.com . He has two books, one for physicians, second for consumers. Good bit of material posted which can be searched.  
 
OUCH could usefully add this material as a PDF file on their site.
