Survey paying $10

While researching pharmaceutical companies to seek funding and support and sponsorship from for OUCH, I came across a survey from Bayer that pays $10 in Amazon gift certificates.
 
Please go to the site and fill out the survey (the survey window pops up seperately) and when you receive your money donate to OUCH and perhaps we can purchase some literature for the site.

Hi Cat,
Just filled out the survey. I'm not sure how you receive the certificate but if its emailed, I will just email it to you if thats ok. (my printer is not working) Just a little confused how it works.
