Letter to employer

Can anyone point me in the right direction to a letter for employers that was posted several months ago? I looked briefly but thought it might be easier to just post the question. Thank you!

It is up to YOU to educate yourself and then help your doctor plan your treatment. If you just sit down in front of your doctor and say "make me better" you are setting yourself up for a great deal of pain.
