$25 for after hour ED services

Well, I hate to bring you the bad news but if you visit your hospital's emergency room department between the hours of 10:00 PM and 8:00 AM, the hospital may tack an additional charge onto your bill.
 
We have been instructed to use CPT code, 99053 for "services provided between 10 PM - 8 AM at a 24 hour facility in addition to basic services".
 
The RVU (relative value unit) for the hospital where I work is $25.
 
Not all payors will recognize this charge but it still has to be billed.

Each time I submit this code, I cringe.  Just doesn't seem fair!

Ohhh so not only do the Doctors and nurses get paid to be there to Ummmm   uhhhhhhh be there, the patient pays a premium if the staff actually has to....WORK?
 
I should tell that to my HR department.  
 
Carol, hi I just wanted to let you know that I want to alter the terms of my employment slightly.  I'll still be here from 8 to 5, you can still pay me my regular wage.  However, if I have to perform any work  I require a premium of 25 dollars an hour in addition to my base wage.
 
I think that would go over swimmingly don't you???

I know.  I am not happy about it either but we have to do what we are told to do.
 
I guess they figure that the emergency dept. staff who work third shift get shift differential pay in addition to their regular pay so the hospitals are trying to recoup that $$.

I know it's hard to find people who work 3rd shift, but the difference in pay must now be substantial, if they are billing $25.00 per patient. Gotta pay for it somehow.

I've been paying my hospital $25 a month.....sometimes more toward an old hospital bill that started at close to $2,000. It's down to $775 now. You can do that with my local hospital. I did it years ago as well to a Baltimore hospital that was incredibly nasty to me when I had to use their ER for a gall bladder attack. Cost a a fortune, even in 1983.

Will insurance pay the additional or will copays now be $25 more? We won't shoot the messenger, Cyn. Thanks for heads up! If we get in before 10pm but aren't discharged until after 10pm, do we pay the extra?

I work on the front end of the billing process so don't see much of how the payors reimburse the hospital.
 
I don't think the co-pays will increase.
 
I can't speak for other hospitals as to whether or not they will be billing the 99053 code in addition to the basic service emergency dept codes of 99281 - 99285 or critical care codes, etc.
 
For us, if you are seen before 10:00 PM and your treatment carries thru past the hours of 10:00 PM, we will not bill the 99053 code.
