Where are the polls and surveys?

I see people referencing polls of smokers, or drinkers, or people with different ailments......where are these stats at?
I would like to see if there are any polls for teeth problems, tmj, blood pressure, diet, water intake, and maybe some other things. Someone quoted a thyroid poll once, where is this stuff at? Anyone ever do a poll on eyesight before?

I have quite bad eyesight and have pretty recently found out I have steroid induced cataracts too although that is NOT related to CH.

been wearing bi-foculs since age 11.
astygmatisms (sp?)
wear prisms to bend one eye 9 degrees.
legally blind in both eyes, although sight has improved to just 20/320
partially color blind (this is what precluded me from gettin my pilots license)
have almost no pigmentation in my eyes.

oh, and PS, the modern terminology is not 'sportin glasses', the new terminology is "pimpin glasses"

Been in glasses or contacts since the fourth grade (won't tell you how long that has been). However, most of my family is equally blind - I'm the only one with cluster syndrome.
