What is the goal or intent behind all the questions on the insurence forms ? Brad's insurence is changeing AGAIN....soon too....the other one that fineally kicked in paying some is up soon and there increaseing there rates 33 %. So the shop is switching again. I have two sets of forms to fill out......they all ask if you have certain conditons. Then they askif you have been treated for any of them. Then they want the meds you were treated with....now or have the past 5 years. Then the next question is what you are takeing or have been the past year up to current. Whats to know 1st  date treated......last date treated......current status and date diagnosed. Also any outpatient stuff like 'testing ect......I put routine blood test cuz it was testing ???? Trick question maybe ?? I am gettin SO confused......I dread doin these.....I HATE them. I am tryin to list meds.....but......will I get into trouble if I get somethin a tad off. I have a copy of the old policy and what we had listed but things have changed and there asking difrferent questions. What do they mean by TREATED ? I've got meds stashed I have never had to use but were scripted so should I list them also ?? I don't want to look like a lier and them get rejected. There's not enuff space for much detail either at all. So many questions seem to be on thee double. I hear our new deductable will be $ 2,000.00 per person and we will have to kick in the first 4 grand before it pays JACK SHIT. This is gonna HURT......this is REALLY REALLY gonna hurt. I am grabbin up all the imitrex I can the doc got him a script to for the next year before the old insurence expires to stock up. Should I try to get in my last MRI before it expires for my back I wonder ? I'm ready to run away Pam

take a deep breath and EXHALE!  When I get home tonight you can send me any questions you have.  I work for an insurance company and can help you fill out the forms.  I will PM you when I get home tonight.

Cool. I did kinda already fill em out the BEST I could tho cuz I mite have to run em out to the shop yet today since due today. But I could use some advice for further issues with these QUESTIONS and how to properly answer. And there may be other forms if they are applying to other places. I either put too much or too less info on em.....but I don't understand what some of them "REALLY" want to know. Thanks.......forms from Mars Pam

Pam thanks for asking questions that I will probably need answered soon. My hubby changed jobs and new ins will kick in on Jan 1st. I haven't gotten any forms yet, but Karen could you walk us through some of it? I'm terrified that it won't cover my CH since it's a pre-existing condition. What do you think will happen Karen?

it's better to put too much information.  This almost sounds like they are trying to get preextisting information and a lot of insurance companies will allow claims to go through if you disclose everything.  Is there a phone # for the insurance company that you can call and ask questions?

Answer the questions honestly.  Under the H.I.P.A.A. regulations you and Brad won't be subject to pre-existing conditions as long as there is no significant lapse in coverage (I believe it is 60 days but I don't have time to look it up right now.)  
 
Find out if the new policy has a deductible on Prescritption Meds or if it has a Drug card that requires only a copay. Might save you some money on stocking up (although that is never a bad idea).
 
If your deductible is projected to be 2 grand...Get the MRI done now.  Otherwise you will likely end up paying for it out of pocket with your new carrier.

Did you hubby have to wait 60 or 90 days before the insurance kicked in?  Were you seen, treated, diagnosed or advised of any condition during that time?
 
If so, they may consider it pre-existing.  There are some ways around that though...check your PM's.

GREAT advice Donna!  I wish I was home and I would call Pam!  As long as there is 18 months credible coverage prior to this policy and there isn't a lapse of more than 60 days there should be no prex - especially with a group policy.  A lot of group policies don't have prex to begin with so I am wondering if she is doing extra work but not a bad idea to fill it all out.  Be very honest and up front and you shouldn't have a problem.

Thanks guys.......maybe some of this will also help some others. We've had no lapse in insurence......there just switching cuz the previous one still in effect is sky rocketing when it rolls over I think they call it ? But I jus don't understand EXACTLY what they mean by giveing them dates for treatments for everything cuz I already had to mention meds and dates. Not sure what 'last treament' means ? The one set of forms wants to know what we are takeing as of THIS DAY. I don't take stuff daily except to sleep......Brad's still out of cycle for now. So I put about everything I could think of on there jus in case.....hopein I'm not doin HARM ? It's simple yet 'not' fillin that stuff out when they seem to be askin the same questions in different WAYS several times. Rite how they are applying to these new company's to beat the others costs so not sure of any benifits yet if ANY other then a very very high deductable. My copay went up with the last switch.....so imagine this will be worse. I did not put anything when it asked about any surgery porcedures your doc mentiond or discussed as possible in the future ??? My doc DID mention epidural's and back stuff but he is not a sugeon and we just talked about it. I hope I don't have to mention that on the forms ?? I have no intentions of it rite now anyhow.
